This introductory course is aimed at practitioners wishing to train as Link Workers/Social Prescribers/Care Connectors prior to applying for a role with local PCN (Primary Care Networks)/GP Practice. As well as for existing practitioners wishing to develop their skill set and learn innovative engagement approaches such as Personalisation, Active Listening, Motivational Interviewing, Diversity Assurance, Health Coaching and Relationship Building.

  • Gain knowledge of what Social Prescribing is and how to implement it in their own surgery/organisation/settings.

  • Understand when it is appropriate to signpost or refer clients to other agencies.

  • ·Demonstrate good practice when referring or signposting clients (Safeguarding, GDPR, Surgery Policies and Lone Working).

  • ·Know how to monitor and evaluate client referrals to other agencies.

  • ·Identify barriers to the approach in their own settings and plan for solutions to these barriers
